This consent explains telehealth care through RX GLEAM. By checking the consent box during intake, you acknowledge you have read and agree to the following.

Nature of telehealth

Telehealth uses electronic communication to deliver care without an in-person visit. Your provider reviews the information you submit and communicates with you through the platform. Care is limited to what can be safely delivered remotely.

Benefits & risks

• Benefits include convenient access to licensed providers and coordinated fulfillment.
• Risks include that information transmitted may be insufficient for a full assessment, and that technology may fail. Your provider may determine telehealth is not appropriate and recommend in-person care.

Your responsibilities

You agree to provide accurate and complete information, to follow your provider's instructions, and to seek in-person or emergency care when needed. You confirm you are the person creating the account and are physically located in a state where we operate.

No guarantee of treatment

Completing an intake does not guarantee a prescription. Treatment is provided only when a licensed provider determines it is clinically appropriate.

Consent to care & e-signature

By checking the consent box and typing your name during intake, you provide your electronic signature and consent to receive telehealth services, to the platform's Terms and Privacy Policy, and to secure electronic communication with your care team. You may withdraw consent at any time by contacting support@rxgleam.com, which may end your ability to receive services.
