RX GLEAM PLLC and the providers delivering your care are required by law to maintain the privacy of your protected health information (PHI), to provide you this notice, and to abide by its terms.

How we may use & disclose PHI

For treatment (sharing with your providers and pharmacies), for payment (processing charges for services), and for healthcare operations (quality, administration, and security). We may also disclose PHI when required by law.

Your rights

You have the right to inspect and obtain a copy of your PHI, to request corrections, to request restrictions on certain uses and disclosures, to an accounting of disclosures, to request confidential communications, and to receive a paper copy of this notice.

Safeguards

We maintain administrative, technical, and physical safeguards, including encryption, access controls, and audit logging, and we require vendors handling PHI to sign Business Associate Agreements.
